A 46-year-old man with long-standing, poorly controlled diabetes is hospitalized for right knee monoarthritis after trauma. His clinical evaluation found joint deformity, synovitis in the and absence of multiple toes due to a history amputation. With radiological that showed bone destruction synovial fluid analysis ruled out infectious causes microcrystal arthritis, diagnosis Charcot neuroarthropathy was made. Conservative management indicated use felt insole unloading walker, case being followed up define behavior according evolution. Despite rare, it important consider all diabetic patients knee, order make an early avoid complications sequelae, always ruling arthritis microcrystals.
